Request for Change from Weekly to Monthly Care Summary

I hope this message finds you well.

I’m writing to request a change from the weekly care summary to a monthly care summary instead. The current weekly summary isn't as helpful for us, and the Illinois Department on Aging is requesting that we submit a monthly summary (Timesheet) with the client and caregiver task and signature. Switching to a monthly format would save time, reduce administrative burden, and provide more space in our physical files. Additionally, there are several fields included in the weekly summary that we do not require. 

Such as payer name, client type, agency info, total billable hours instead you can put total hours , and weekly comments, answer each question by checking yes or no, additional comments, and the note there's no need for them. Instead of the RN's signature, you can put the supervisor's and caregiver signature. 


I have an example please see the attached the highlighted in yellow means add fields and whatever in pink you can  deleted fields

When a caregiver leaves a note about a shift, it would be convenient for that to show on the Task/Communication tab of the client file for easy access.

Once we have selected the Clients tab on left of screen, just as we are able to select clients for a particular territory.  We should have the ability to toggle a selection for a specific Client type or Payer also to further breakdown our selection of listed clients.

Mgmt alerts when an Incident Report is submitted

Until there is a way to generate a report on Health and Safety incidents, have a management email alert when a new incident form has been created.   That would save A LOT of time, because right now we have to go back through each profile to pull the information, and track it outside of Caresmartz, in an Excel sheet :-(    Not efficient!

For quality control and reporting requirements (government and accreditation) we need a way to generate reports on number and types of incident reports files. In fact, it would be helpful to have that on the management dashboard.  By incident/injury to Client, by incident/injury to Caregiver, by type of incident, and frequency.    Would rate this as a very high priority.

Good news! This function is already in place. 

To control your Careiver application settings, navigate to the settings>>Caregiver>>Caregiver Applicant Settings

From there, you will see a section called "Miscellaneous Questions". There are then three settings 1 - Enable/Disable 2 - Is Mandatory? - this is where you can make one or more questions required. 3 - Mandatory Count - this setting allows you to choose the number of custom questions you want to be required. If you have three questions, set this to 3.

Notes on Weekly Care Summary

On the Weekly Care Summary report, the Visit notes are not included. Visit Notes are mandatory as the narrative on Written Progress Report for Medicaid. Please add visit notes to the Weekly Care Summary and remove Vitals
