customizable job application

Please allow for more customizable fields on the job applications. The calendar tool is causing many problems for our prospective employees - some of them are not filling out forms because of it and I can't turn it off.

The document attachement form doesn't make sense, I'd like to remove it from former employment. 

I would like to remove birthdate (I disabled but can't remove) and I created a separate field below because of the calendar tool not working properly. 

Further customization of the fields and existing items would be appreciated. We are losing applicants because they are unable to fill out the application and giving up.
